CCS consumers receive individualized direct supports (on the job and throughout the community) from our dedicated staff of Direct Support Professionals (DSP). Some also receive home and shared living supports through CCS.

The service coordination team at CCS takes responsibility for managing the entire array of services we provide each consumer.

Our approach to service coordination emphasizes:

  • Ensuring each client’s health and safety
  • Developing a team approach
  • Clear and concise communication
  • Maintaining healthy and effective working relationships with consumers and their families

Our team works together with each consumer, and her/his family and other natural supports to develop an annual Individualized Service Agreement (ISA). The ISA reflects goals and objectives for the year, and serves as the guide for each consumer’s specific schedule and services.
